The Postal Service Form 1583 is in PDF format. To view and print a PDF file you must have the FREE Adobe Acrobat Reader 3.0 loaded on your computer.

Version 3 of the Acrobat Reader provides a major improvement over previous releases of the software. When installed as a plug-in to your browser, it gives you one page-at-a-time delivery of pdf files and lets you use cross-document linking (bookmarks and internal document links). You no longer have to wait for the complete document to load before you can use it. The plug-in displays the PDF document inside the Netscape or Internet Explorer (see note below) window rather than in a separate application.

To install the Acrobat Reader as a plug in, install Netscape 3.0 (or higher) or Internet Explorer 3.0 (or higher) first, then install the Acrobat Reader 3.0. Remove any existing application definition for PDF files and close your browser. When you restart the browser, it will recognize the Reader as a plug-in.

NOTE: Some versions of Internet Explorer will not work properly with Acrobat Reader 3.0. Adobe has provided a patch to allow these version of the browser to correctly load PDF files. If you are unable to view PDF files in Internet Explorer, follow these steps:

1. Select the link to Adobe on the right.

2. Select the "Download" button from the Adobe site

3. Select "Important information for Microsoft Internet Explorer 4.0 Users". While this link refers to version 4.0, we have found that the information may also be necessary for earlier versions of Internet Explorer.

4. Select the download option at the bottom of the page.

5. Download the file oxcinst.exe and follow the directions for installing the ActiveX update.

6. When you restart your computer, you will be able to view PDF files within the Internet Explorer browser.

Complete Postal Service Form 1583

1. List all names in which mail will be sent in block 2.

2. Leave blocks 3 and 4 blank. We will complete these for you.

3. Fill out as much of the application as you can, and if you should have any questions, you may contact us and we will assist you with completion.

4. The postal service requires copies of two pieces of identification from each applicant accompany the form 1583. One identification must have a picture, such as a passport, driver license, alien registration, government issued photo ID, or military ID. The other piece of identification must have the applicant's name and a corresponding number, such as a voter's registration, library card, insurance card. Credit cards and social security cards are not acceptable.
