HEALTH STATUS:
I am in excellent health in body and mind. I exercise for a minimum of thirty minutes daily. I prefer swimming. I eat primarily low fat foods with a diet consisting primarily of fish, vegetables and fruit. I do not take any medications on a regular basis. I take vitamin, mineral and ginseng supplements daily. I drink several cups of fruit juice, soy milk, green tea and black tea daily. I do not smoke and I do not use illicit drugs. I do not drink alcohol products. I meditate daily. My mental health care status is also excellent and is as follows: I have always been ambitious,outgoing and a self-starter. I do not suffer from mental illness and I have never suffered from mental illness. I am a conservative dresser. I lead a quite family life and spend my free time with my wife and children. I do not engage in extra-marital affairs. I am heterosexual and in strong support of traditional marriages between men and women only. I do not suffer from auditory, tactile or visual hallucinations. I do not have suicidal or homicidal ideation or intent. I sleep well at night. I am a conservative gambler. I do not suffer from racing thoughts. I do not have any criminal intent in the pursuit of my interests and I have no criminal convictions.
