When Kirsty Cotton awakes, the nightmares do not
stop. The memories of her dead father, and her evil uncle and step-mother, are vivid in her mind. Now, suffering from trauma, she is a patient at a psychiatric hospital for the mentally disturbed.
Despite being at the peak of his profession and
exuding an air of competence and power, Dr
Channard, head of the hospital, conceals deep and
dark secrets. Listening to Kirsty's story, he is now keen to discover the secrets of the puzzle box - also known as the Lament Configuration - which opens the door to ultimate pleasure or ultimate pain.
During a police investigation at the house where the grizzly murders took place, a police officer receives a paper cut and spills some of his blood on the mattress where Julia died. Seeing the blood disappear almost immediately, Dr Channard convinces the police to deliver the blood-stained mattress to his house. With a chance to resurrect Julia from the dead, Dr Channard is prepared to go to any lengths to unlock the portals that lead into the Outer Darkness.
A vision of Kirsty's murdered father, Larry, manifests itself in the young woman's hospital room and writes a message on the wall, begging her for help. Left with no choice, she must venture again into the horrifying Stygian corridors of the Cenobites to stop Dr Channard, kill Julia and release her father from his eternal torment.

Seeking the ultimate in sensual ecstasy that he thought he would receive, he gets torn apart, limb from limb, by some hideous demons instead.
Many years pass, and Frank's brother Larry moves
into the same London house with his family. While
moving furniture one day in the upper room, Larry
severely injures himself. But his wife Julia, who feels that their marriage is all but over, sees something spooky - the blood that had spilled on the ground starts evaporating.Some time later, Julia finds a horrifying remnant of Frank in the room. Coming back from the dead, he regenerates by feeding on human blood and flesh. Julia, who had a torrid affair with Frank, seduces men, kills them in her house and feeds them to Frank. But the dreadful couple did not count on Larry's daughter, Kirsty, to take revenge on them. Summoning the demons from the box, she makes a bargain with them in exchange for her freedom. But the dreadful monsters do not care who appears in front of them - all they want is human flesh, and a lot of it.
In danger of losing her own life, Kirsty must escape the wrath of hell's angels and turn them onto Uncle Frank - before she gets dragged into Hell's horrifying Stygian corridors.

Pinhead - the Black Pope of Hell - is released from his exile in an ancient pillar. To show his 'gratefulness', he makes a deal with the pillar's owner - the pleasures of flesh, power and dominion, and in return, the sacrifice of innocent victims to the pillar.
J.P. Monroe - rich, spoiled New York nightclub owner - becomes transfixed by a mysterious six-foot tall pillar he sees in an equally mysterious museum. Amongst the writhing figures and distorted faces etched into its surface are the marbled yet distinct features of the evil Pinhead. Deciding that this bizarre artifact will be the attraction in his nightclub, Monroe buys the pillar. Meanwhile, Joey, a bored young reporter, is just wrapping up another non-news story in the local hospital when paramedics wheel in a semi-conscious young boy fighting for his life. Dangling from his face, body and limbs are long and bloody hooks and chains. Before anybody can make a move to help him, the chains, as if tugged by an invisible hand, pull and tear the boy apart, scattering his bloody flesh in all directions.
Excited by the gruesome murder that she just
witnessed, Joey traces the murder to the boy's
girlfriend, who reveals that the chains that killed her boyfriend came from a mysterious box that the boy found on the pillar.
In the club, Monroe accidentally cuts himself and spills a few drops of blood on the pillar, releasing the imprisoned soul of Pinhead. The hideous monster, grateful for being released, grants the hedonistic nightclub owner with the ultimate pleasure, in return for a regular sacrifice of innocent victims to the pillar.
