There are many placements for elder loved ones, including assisted living, retirement communities, group homes, and skilled nursing facilities. None of them can provide the personal care of a loving family, but there is an alternative solution that will help you better understand the care that your loved one is entitled to. Eldercare will provide you with the resources needed to acquire the better care your loved one deserves.
Eldercare provides assistance with finding the proper facility best suited for your loved one, in consideration of eligibility, financial resources, and legal concerns. Onsite interviews with the facility staff, and an extensive review of the facility's history, are performed to ensure the services offered are sufficient for your loved one's needs. You will also receive assistance with the admission and application process, as well as developing initial and follow-up care plans, defining each of your loved one's needs, and how the facility can best suit those needs.
Once placed, or if already placed at time of contract, Eldercare will ensure that your loved one continues to receive the best of care by monitoring the facility with periodic and unscheduled weekly visits, and alert you to, and assist with correcting, any concerns that are discovered.
Eldercare will attend all care plan meetings concerning your loved one, and will review and assess all care plans implented by the facility, and perform follow-up assessments to ensure that the care plans are being adhered to.
Comparitive assessments will also be performed to ensure that your loved is being assessed properly. Regular scheduled screenings will be performed for your loved one concerning cognitive & memory functioning, dementia, and depression, to identify symptoms early, to offer the best possible outcome. Referral and liaison services will be provided for geriatric care specialists including dentists, elder lawyers, estate planners, financial planners, guardians, home health services, skilled geriatric nursing services, physicians, psychiatrists, and psychologists.
Advocacy, education, and support will also be provided for your family and loved one to better understand and address concerns. Crisis intervention is also provided with each contract to address any sudden or unexpected concerns that may arise.
