  3. Send a copy of your driver's license and one other form of I.D., such as a Military or other Government ID, Passport, Alien Registration Card, Library Card, Voter's Registration Card, or another identification that has a number corresponding with the individual person.


  4. Once your application forms, I.D. and payment are received, we will you assign you your unique mailbox number. We will contact you within 2 days - by telephone, fax or e-mail and confirm your new address, which will be:

They will give you a FUND TRANSFER NUMBER, send it through e-mail and we will collect the money within 24 hours.

This contract is an agreement giving my (our) authorization for BJ Mailing Service to receive and accept as my/our agent any mail or packages addressed to me/us delivered and handle it according to instructions. I hereby release BJ Mailing Service from any liability for loss or destruction of mail unless the same was caused by gross negligence of BJ Mailing Service. BJ Mailing Service will not be liable to incidental, special, or consequential damages. This form constitutes the entire agreement, and supersede any other written or oral agreement. Charges for services rendered by BJ Mailing Service will be payable in Wasco, Oregon. I understand that should I become delinquent, BJ Mailing Service may cease to forward my mail, and after reasonable notice to me, may return all mail to the sender.
